This form/data was expected for this visit. Form 92 (Pap Smear) was expected at Baseline and at Annual Visits 3, 6, and 9 for all HT participants with a cervix prior to study closures. Pap Smears were not expected for E+P after study closure on 07/08/2002 or for E-Alone after study closure on 02/29/2004.

Usage Notes:

This includes forms from the subsample of OS participants at year 6 that were enrolled in the three clinical centers participating in the WHI Osteoporosis sub study, as height and weight were collected on OS participants at year 6 by design; see table 16.1; on page 16-18b of volume 2.
